Question by daisy: If you go into therapy/see a psychologist, does this go into your medical records?
I do never take drugs, even though I was advised, so I’m just wondering if the standard of care should go into medical records? If it is, what exactly is the disc? An official diagnosis, until going into detail?

Medical records are whatever that particular doctor or therapist writes in your file. Each practitioner you see has their own file & they don’t share them without your permission. There is no big master file that contains all your documents in one place. And they are confidential by law.
I have been into doctors, psychiatrist, & therapists but they don’t communicate with each other.

Your diagnosis and medication(s) will be on your records because it’s relevant information to all of your current and potential doctors. The details of what you talk about with your therapist, however, are completely restricted and no details of any of that will be available to anybody but you and your therapist in normal circumstances.
You will however be reported if you are considered a risk to yourself or others. That is not confidential and will be dealt with accordingly.

Only if you tell one of your other doctors about it. Cause the therapist or psycologist isn’t M.D. So they don’t have medical records. only notes.
